"We only pay the CRO when they hit the milestone." I often hear this framed as oversight. I see it differently.
Milestone based payment measures whether a task was completed. It does not tell you whether the trial is being managed well. It does not tell you why the same issue keeps reappearing every week, whether timelines are slipping because of weak execution or unrealistic resourcing, whether a site needs another study coordinator, whether the CRA is overloaded, or whether the protocol was ever made truly usable for the people expected to follow it.
And when payment becomes the main control mechanism, people focus on protecting the metric rather than openly surfacing what is actually going wrong.
Real oversight is not about making vendors work under fear. It is about understanding what stands between the trial and success, and fixing it. Sometimes that means escalation. Sometimes stronger accountability. Sometimes more support, clearer processes, better communication, or additional resources where they are genuinely needed.
The goal is not to catch the CRO. The goal is to make the trial better. Better managed, better supported, better controlled, and far more likely to deliver reliable results.
